iS it a bug?

I was trying to fill slots in my extreme fatal helm, i filled 1st slot with cr 2nd with cd and I wan to put cd to the last slot. Item is fully upgraded to +15 with 99% succes rate and I just had 5x fails in a row. I am doing something wrong or Im just unlucky as hell?

Now, that we managed to forge an Extreme Outrageous Mithril Blade with 2 Epic Forces and 3 empty slots, lets fill the slots with some force.
As mentioned before adding slot forces to an Extreme Crafted item is not the same as the normal Craft Items.
Extreme Crafted items have some limitations regarding the slots:

• These items can either have 2 of the same force and the 3rd one with a different force or 3 of the same force.
• The first slot force you add to the item will be the one that can occupy a second (double) slot.
